This Week’s Worst Health Inspection Violations

Each week going forward, we intend to bring you a list of 5 restaurants with the largest number of health inspection violations.  This week’s worst offenders had violations that included, among other things a whopping 38 roaches in one restaurant, touching raw eggs and meat and then touching ready to eat foods, and lots of food not held at a safe temperature.

Star’s Caribbean Restaurant
Address: 1901 Blanding Boulevard
Violations: Total: 18, High: 2, Intermediate: 5, Basic: 11

High Priority – Potentially hazardous (time/temperature control for safety) hot held at less than 135 degrees Fahrenheit or above. 125-130° chicken on steam table, employee placed it in oven to reheat, 88° plantains on steam table, fried again, explained time as public health control, **Corrective Action Taken** **Admin Complaint**

High Priority – Ready-to-eat food contaminated. Cooked sliced pork cooked on smoker outside **Warning**

Osaka Grill Sushi Buffet

Address: 11701 San Jose Blvd. Suite 3

Violations: Total: 18, High: 3, Intermediate: 3, Basic: 12

High Priority – Container of medicine improperly stored. Sore throat medicine stored on shelf over reach in cooler. Person in charge removed medicine from shelf. **Corrected On-Site** **Warning**

High Priority – Raw animal foods not properly separated from one another based upon minimum required cooking temperature in walk-in freezer _ all products not commercially packaged. Container of raw chicken stored over case of raw beef inside walk in cooler. Person in charge placed raw beef over raw chicken container. **Corrected On-Site** **Repeat Violation** **Warning**

High Priority – Roach activity present as evidenced by live roaches found. 6 live roaches found under beer reach in cooler near compressor unit. 6 live roaches found behind upright reach in cooler near cook line. 1 live roach found behind upright reach in cooler near door entrance to dining area. 25 live roaches found behind reach in cooler on right wheel on cook line. **Warning**

A follow up inspection took place and the following was the only violation:

High Priority – – From inspection on 2016-02-16: High Priority – Roach activity present as evidenced by live roaches found. 6 live roaches found under beer reach in cooler near compressor unit. 6 live roaches found behind upright reach in cooler near cook line. 1 live roach found behind upright reach in cooler near door entrance to dining area. 25 live roaches found behind reach in cooler on right wheel on cook line. **Warning** – From follow-up inspection on 2016-02-17: 1 live roach found on bag of bread crumbs near rear kitchen door. 1 live roach found on gasket at reach in cooler on cook line. 3 live roaches found behind upright reach in freezer on cook line. 2 live roaches found behind reach in cooler on cook line. 1 live roach found on box bag soda near rear kitchen door. **Time Extended**

Waffle House

Address: 6796 Commonwealth Ave.

Violations: Total: 19, High: 5, Intermediate: 8, Basic: 6

High Priority – Employee switched from working with raw food to ready-to-eat food without washing hands. Grabbed raw bacon, and sausage patties then went to get grits from back warmer with same gloves on, and another cook cracked eggs and grabbed cheese for the grits with same gloves on, explained to them to wash hands and they did **Corrected On-Site** **Repeat Violation** **Warning**

High Priority – Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. 46° ham in drawer unit, closed package, employee iced down **Corrective Action Taken** **Repeat Violation** **Warning**

High Priority – Potentially hazardous (time/temperature control for safety) hot held at less than 135 degrees Fahrenheit or above. 95° grits on top of warmer, mgr placed them on grill to heat up **Corrective Action Taken** **Warning**

High Priority – Wiping cloth sanitizer solution exceeds the maximum concentration allowed. 300 ppm chl in bucket, employee diluted it to 200 ppm **Corrected On-Site** **Warning**

Violations: Total: 20, High: 7, Intermediate: 2, Basic: 11

High Priority – Potentially hazardous (time/temperature control for safety) food cold held at greater than 41 degrees Fahrenheit. sausages 51-52°, cheesecake 51-52° **Repeat Violation** **Warning**

High Priority – Potentially hazardous (time/temperature control for safety) hot held at less than 135 degrees Fahrenheit or above. Soup 126° in server area, recommended turning up temperature of soup kettle **Warning**

High Priority – Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Quat sanitizer less than 150 ppm, instructed manager to use dish machine to sanitize instead **Warning**

High Priority – Raw animal food stored over or with ready-to-eat food in reach-in freezer _ not all products commercially packaged. Container of Uncooked frozen breaded chicken and container of uncooked steak stored over top of container of potatoes in reach in freezer, cook corrected during inspection **Corrected On-Site** **Warning**

Phil This Soul Food

Address: 5534 Highway Ave.

Violations: Total: 20, High: 7, Intermediate: 5, Basic: 8

High Priority – Employee cracked raw shell eggs and then handled ready-to-eat food and/or clean equipment or utensils without washing hands. Cracked eggs then grabbed bread with same gloves on, explained to her and she washed hands and change gloves **Corrected On-Site**

High Priority – Nonfood-grade containers used for food storage – direct contact with food. For chicken wings, toys on label

High Priority – Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. 0 ppm, read directions on quat sanitizer and employee stated she doesn’t put that much in triple sink, employee set up triple sink with bleach because they don’t have enough quat sanitizer

High Priority – Raw animal food stored over ready-to-eat food. Raw eggs on shelf above cabbage, upright cooler, mgr placed eggs on bottom **Corrected On-Site**

High Priority – Shell eggs held in a unit maintaining an ambient air temperature greater than 45 degrees Fahrenheit. On table, 60 ° eggs, mgr placed them back in cooler, explained time as public health control, **Corrective Action Taken**

High Priority – Shell eggs in use or stored with cracks or broken shells. Mgr removed two cracked eggs, **Corrected On-Site**

High Priority – Toxic substance/chemical stored by or with food. Cleaner by food on shel in middle table, mgr removed chemical **Corrected On-Site**
